selected, the “Dates, Values, and Units of Measure” fields do not apply. Although the “Dates, Values, and Units of

date fields.
Lab:
The list displays lab tests specific for
the “Lab Category“ selected.
If an appropriate lab
test is not included in the list, enter it in “Other Lab” field.
When
entering information regarding a Lab not listed, only the “Other Lab” field is entered. The Lab section cannot be saved if data
is entered into “Other Lab,” “Lab Category,” and/or “Lab” fields.
Baseline,
Nadir/Worst, and Recovery or Latest
Values for “Baseline,” “Nadir/Worst,” and “Recovery
or Latest” are mandatory for all Labs
(except Microbiology).
Occasionally, “Baseline,” “Nadir/Worst,” and “Recovery
or Latest” might be the same. For
example, a baseline value may not be available for an abnormal lab that
occurred during treatment. In this case, the baseline value and date entered
would be the first lab obtained.
Summary of possible date combinations:
·
Baseline is equal to or less than Nadir/Worst and/or
Recovery or Latest
·
Nadir/Worst is equal to or greater than Baseline; is equal
to or less than Recovery
·
Recovery or Latest is equal to or greater than Baseline
and/or Nadir
